Cherokee Nation Extends School Clothing Assistance Program

Beginning July 22, 2019, the School Clothing Assistance Program will begin accepting clothing applications again through July 31, 2019. The downloadable application can be found on the downloads section below or can be picked up at any of the Human Services field office locations. Once you have filled out the application, you must submit the original by mail to:

or walk into any of the field office locations (Faxed or Emailed applications will not be accepted this year) The application process ends at 5 pm on July 31, 2019. Mailed-in applications need to be postmarked no later than July 31st.

ELIGIBILITY

  • Student MUST be a Cherokee Nation Citizen – copies of tribal cards
  • Must live within the Cherokee Nation Jurisdictional Area – this will be verified by a current utility bill, not older than 30 days, that shows your physical address or service address that you will include with your application.
  • Student must be in grades K – 12 for the upcoming school year. Pre-K is NOT eligible for this program.
  • Kindergarten students must be age 5 before 9/01/2019
  • Proof of school enrollment for each child (copies of enrollment, letter from school, or last semester report card)
  • Applications must be made by custodial parent or legal guardians & must provide copies of their I.D.
  • Guardians must provide copies of Letters of Guardianship issued by a District Court. If this documentation is not available, the following will be accepted: “proof of residency” letter on school letter head from a school official verifying the said guardian on the application is listed as the guardian or a copy of the school enrollment packet that identifies applicant as guardian of child(ren).
  • Copies of income verification for ALL members of the household. (income tax forms, W2’s or last check stub of previous year).
  • Household must meet income guidelines. Income guidelines are the 2018 National Poverty Income Guidelines plus 25%.
